Providing community services
The transfer of Primary Care Trust (PCT) run community based services out to a range of different organisations presents Assura Medical with an opportunity to deliver services by building on our experience of providing innovative, cost effective and integrated community services.
Our approach to delivering NHS services through working in partnership with primary, secondary, community, independent, 3rd sector and social care organisations enables us to deliver the wide range of community services formerly provided by most PCTs.
To meet the financial challenges facing the NHS and to also ensure patients receive joined up, seamless care, irrespective of who has responsibility to deliver it, requires organisations across the health and social care sectors to work in an integrated way.
Community services are often ‘the glue’ which joins primary care to secondary care, and also provides the critical link across the health and social care sectors. Unless these services can be delivered in a truly integrated way, gaps appear for patients and expensive duplication occurs between organisations.
Assura Medical has developed a range of community based pathway services that bring together a wide range of organisations to provide patients with seamless care that removes duplication and is therefore more cost effective for commissioners.

Assura Hampshire Health MSK service.
Operating in partnership with two acute providers and providing a comprehensive community based service involving, GPs with a specialist interest (GPSi), consultants, extended scope physiotherapists, community physiotherapy, podiatry and diagnostics within the pathway.
Assura Reading Dermatology service.
Provided in partnership with the Royal Berkshire Foundation Trust and offering a range of full dermatological procedures including community based assessment and treatment through to hospital based services including suspected cancer and two week waits.
Assura Stockton Sexual Health service.
A comprehensive community based sexual health service provided by rationalising the number of service providers from four trusts into one organisation to deliver the service across four PCTS.
